WakeMeds team of more than 8,200 employees, 250 employed physicians, 1,500 volunteers, 1,200 affiliated physicians use the most advanced technologies along with the best minds and the biggest hearts to ensure the finest quality in health care and community health Position Information The Home Health Care Coordinator is a Registered Nurse who is assigned to WakeMed hospitals, to assist with facilitating the plan of care for an inpatient episode and timely discharge to Home Health by using a collaborative interdisciplinary approach. Works in collaboration with the hospital discharge team to anticipate the needs of the patient and the services necessary to meet those needs. The Care Coordinator works collaboratively with the case management department and other departments, in assessing patient needs and developing the plan of care, once the patient has chosen WakeMed Home Health. The Care Coordinator will also act as an advocate for the patient, physician and hospital relative to compliance with federal, state and third party utilization management regulatory requirements pertaining to home health. Experience Requirements : 1 Year Nursing - Related Area Or 1 Year Clinical - Case Management Or 1 Year Clinical - Utilization Management Required - And 1 Year Clinical - Home or Community Health Preferred Education Required : Graduate Nursing Required - And Bachelor's Degree Nursing Preferred Licensure/Certification Requirements : Registered Nurse Required Hours of Work : weekends/day Weekend Requirements : Every Weekend Call Requirements : as needed
