Job Description:
Liberty Healthcare seeks a dedicated MDS (Minimum Data Set)
Coordinator to join our dynamic team, focusing on enhancing the
wellbeing of our clients through personalized and innovative care.
As an MDS Coordinator, you will ensure the accuracy and timeliness
of the MDS assessments per federal and state regulations to
optimize the quality of care and comfort provided to our residents.
You will be pivotal in interdisciplinary care planning and
maintaining compliance with CMS guidelines, reflecting Liberty
Healthcares commitment to excellence. Coordinating and facilitating
the timely and accurate completion of the Resident Assessment
Instrument (RAI) and Minimum Data Set (MDS) assessments according
to CMS regulations and guidelines. Collaborating with healthcare
professionals to gather patient information and assess the needs of
patients to develop comprehensive care plans. Ensuring the
interdisciplinary team completes specified tasks and assessments
associated with the MDS and care planning process. Training and
supporting nursing staff on the MDS process, care planning, and
changes in regulations. Overseeing the transmission of MDS
assessments to ensure they are timely and in compliance with state
and federal requirements. Analyzing patient outcomes, quality
assurance data, and compliance reports to identify trends and
improve service quality. Collaborating with the healthcare team to
optimize reimbursement through accurate documentation and adherence
to regulatory compliance. In-depth knowledge of RAI process, MDS
completion, and CMS guidelines. Strong analytical skills and
ability to interpret complex patient data. Exceptional coordination
and communication skills to work effectively with an
interdisciplinary team. Proficient in time management and meeting
multiple deadlines. Ability to train and lead staff in clinical and
regulatory frameworks. Medical Insurance Life Insurance Active
Registered Nurse (RN) license or Licensed Practical Nurse (LPN) in
the state of employment. Certification in MDS 3.0 preferred.
